Osakidetza – Basque Country Health System
Osakidetza is the public healthcare service of the Basque Country in the north of Spain. The Basque health system includes 14 hospitals and more than 100 primary care clinics in four geographical areas. Osakidetza employs more than 30,000 professionals and has a target population of more than 2.3 million inhabitants.
Osakidetza has developed guidelines and a model for integrated care for multi-morbid patients, supported by complex information systems such as the EHR, Personal Health folder, ePrescription, and the Multichannel Communication Service. It has implemented several pilots on remote monitoring for chronic patients (COPD, asthma, heart failure and diabetes) and patient empowerment (Active Patient). These deployment experiences will feed into the ACT@Scale project, in which Osakidetza leads WP6. Learn more about the services included in ACT@Scale here.
